Runnymede’s North Western Toronto Ontario Health Team Progresses to Full Application
The Government of Ontario has a vision to build a connected, sustainable healthcare system centred on the needs of patients. Key to realizing this vision is the creation of Ontario Health Teams (OHTs). Within this new model, health care providers will work as one coordinated team to make it easier for patients and our community to receive seamless, integrated and coordinated, patient-centred care.
Runnymede is a member of the North Western Toronto OHT, a collaboration of healthcare providers that cover a broad scope of services including primary and acute care, rehabilitation, complex continuing care, mental health, long-term care and community services. Recently the North Western Toronto OHT was invited to progress and complete a full OHT application. All OHTs chosen to move forward with a full application are expected to progress onto OHT candidates and implement this new model of care.
The calibre of our self-assessment was exceptional. As a result we were one of only 31 out of 150 teams, chosen to be part of this exclusive group. This is an exciting time for Runnymede as OHT candidates will carve out the future of innovative healthcare delivery in Ontario and set the example future OHT implementation.
Establishing our OHT will take time, but we are excited to be able to find more efficient ways to support patients on every step of their healthcare journey.
